Investigations and Management

Conservative Management:

  • Start topical treatments for external hemorrhoids (e.g., hydrocortisone cream).
  • Recommend dietary changes, including increased fiber intake, and prescribe stool softeners to prevent constipation.

Monitor Symptoms:

  • Monitor the rectal bleeding and assess for any worsening or new symptoms. If bleeding persists or worsens, consider further investigations like a colonoscopy.

Patient Education:

  • Advise the patient to avoid straining during bowel movements and to follow lifestyle changes to manage hemorrhoids.

Monitor Liver Cirrhosis:

  • Continue to monitor for potential complications related to liver cirrhosis, such as variceal bleeding or portal hypertension.

Escalation:

  • If symptoms worsen or other concerning signs develop, escalate the case to a senior for further evaluation or intervention.

Further Investigation (If Needed):

  • If the bleeding continues or other symptoms (e.g., weight loss, change in stool appearance) develop, consider further investigations:
    • Flexible Sigmoidoscopy or Colonoscopy: To rule out other potential sources of bleeding such as colorectal cancer or inflammatory bowel disease.
    • Stool Studies: If there is suspicion of infection or inflammatory causes, stool tests for pathogens or occult blood may be appropriate.
