Mr. John Smith is a 67-year-old man with a history of Crohn’s disease, who was admitted with a flare that has now been successfully treated. His condition is stable and he is medically fit for discharge. While completing his TTOs, the nurse approaches you to inform you that Mr. Smith has experienced fresh rectal bleeding. His vital signs are stable, and he has no history of previous rectal bleeding or recent trauma. Perianal disease has already been ruled out. He reports occasional constipation due to Crohn’s disease but denies abdominal pain, tenesmus, or any changes in bowel habits.
